Job Description:

Touchstone Communities is seeking a dedicated RN/LVN Charge Nurse to join our team in providing exceptional care tailored to the unique needs of our senior residents. This position involves overseeing nursing activities, ensuring high standards of patient care, and collaborating with multidisciplinary teams to enhance patient outcomes in our long-term care facilities across Texas. As a pivotal member of our healthcare team, you will play a crucial role in maintaining a supportive environment for our elders, driven by compassion and professionalism. Supervise and manage the nursing staff to ensure compliance with all federal, state, and local regulations. Coordinate and administer comprehensive patient care plans in consultation with healthcare team members. Monitor patient conditions and update care plans based on assessments. Educate patients and their families on disease processes, treatments, and healthcare management. Administer medications and treatments as prescribed and monitor for any side effects. Address and resolve any issues or conflicts within the nursing staff. Maintain accurate and confidential patient records. Participate in training and professional development sessions to stay updated on industry best practices. Strong leadership and organizational skills Excellent communication and interpersonal skills Proficient in clinical assessments and patient care planning Adept in managing multiple tasks in a dynamic environment High level of empathy and patience with elder care Solid problem-solving and decision-making capabilities 401k Valid RN or LVN license in the state of Texas CPR certified First Aid certification Additional certification in geriatric nursing or long-term care highly desirable
